Recuperative care through Community Supports.

Vantage House offers recuperative care for CenCal Health Medi-Cal members, subject to eligibility, individual review, availability, and applicable authorization. Housing deposit assistance and other services are subject to their own eligibility and authorization requirements.

About recuperative care — CenCal Health

*Admission and length of stay depend on eligibility, individual needs, program availability, and applicable authorization. Services are based on each participant’s needs and program availability. A stay of 180 days is not guaranteed.

Plan the transition together.

Discuss ongoing clinical needs with Community Well California before discharge. Home health services may be coordinated as needed when a participant cannot independently complete daily activities, subject to team review and arrangements being confirmed. Ongoing clinical care is coordinated with the treating providers.

BEFORE YOU REFER

Match support needs
to the setting.

These are Vantage House program criteria supplied by our team. They are separate from CenCal Health’s coverage determination. Every referral receives an individual review.

  • Able to safely and independently use medical equipment and transfer to the toilet.
  • Able to walk with or without medical equipment.

PARTNER QUESTIONS

A few helpful details.

Is a 180-day stay guaranteed?

No. Our six-bed home offers up to 180 days of short-term residential support. Admission and length of stay depend on eligibility, individual needs, availability, and applicable authorization.

Can I confirm eligibility or bed availability here?

The fit check is a conversation guide. The Community Well California team must confirm eligibility, whether care needs can be supported, applicable authorization, and current capacity.

What if the individual needs ongoing clinical care?

Discuss specific clinical tasks, the level of assistance needed, and the proposed follow-up plan with the team before discharge. This website does not confirm that clinical care needs can be met.

What if coverage or authorization is unclear?

Vantage House offers this service through CenCal Health Medi-Cal. Select “not sure” in the fit check if membership or authorization is unclear, and call the team to verify. This guide does not make coverage decisions.
